ABOUT CCHI

CCHI is a non-profit, statewide association based in Sacramento and comprised of over 32 community-based organizations, local government entities, and local partners.

Our members specialize in Outreach, Enrollment, Retention, and Utilization (OERU) services for underserved populations throughout the state and focus on addressing social determinants of health to ensure all Californians have access to affordable health care.

Our Mission & Impact

Since 2007, CCHI’s mission has been to improve the health and well-being of California children and families by supporting our members in meeting the needs of their communities.
CCHI currently runs a variety of initiatives throughout the state, including:

  • The California Health Interconnectivity Initiative, funded by the California Wellness Foundation
  • The Community Health Worker (CHW) Initiative, previously funded by Partnership Health Plan of California
  • The Connecting Kids to Care 2025 Grant, funded by the Centers of Medicaid & Medicare Services (CMS) 
  • Empowering Communities with Digital Literacy for Better Health Access, funded by the California Public Utilities Commission
  • The Outreach, Enrollment, Education, Utilization, and Retention (OEERU) Initiative, funded by The California Endowment
